Substance Abuse Evaluation

Purpose

To provide a neutral non judgmental evaluation of your substance use history for the purposes of program placement, severity of addiction or misuse, and likelihood of relapse.

In compliance with court requests or drug diversion program requests, I assess the nature, severity, and impact of substance use to inform diagnosis, treatment planning, and appropriate referrals for medical or higher-level care.

Scope

This evaluation addresses current and past use of alcohol, prescription medications, over-the-counter substances, illicit drugs, nicotine, and other psychoactive substances. It incorporates screening for co-occurring mental health conditions, trauma history, medical risk factors, and social determinants of health.

I consider an individuals current and past environments and trauma, and current mental health status, as part of my evaluation to assess the appropriate level of care.
